Passport Applications are accepted between 8:30 am - 4:00 pm, Monday through Friday except for legal holidays, at the Lafayette Parish Courthouse located at 800 South Buchanan Street, Lafayette, La 70501.


PASSPORT BOOK

A Passport Book is valid for international travel by land, sea and AIR. Passport Book applications are accepted at the Lafayette Parish Clerk of Court Office. In order to obtain a Passport Book, the applicant must have the following and appear in person:


  • Proof of U.S. Citizenship, which is a certified copy of your birth certificate or naturalization certificate and a photocopy. Louisiana birth certificates or birth cards are available in our office. See link to the right for more information.

  • Proof of identity, I.E.; a valid driver's license or government ID card, along with a photocopy of front and back. Out of state licenses will require a second form of identification.

  • One (1) passport picture. For requirements please click HERE.

  • Fees are $110.00 for adults, age 16 and over (valid for 10 years). 16 - 17 year olds must be accompanied by one parent who has proof of identity.

  • Fees are $80.00 for children, age 15 and under (valid for 5 years); see special requirements below for children under the age of 16.

  • Payable by check or money order to: U. S. Department of State

  • A $35.00 execution fee, payable by separate check, money order, cash or credit/debit card is due to the Clerk of Court.

  • Currently Passport Book processing takes 4-6 weeks.

  • If you need to expedite your application you will need to pay an expedite fee of $60.00 for 2-3 week processing. You may pay an optional $16.48 for faster return delivery of your issued passport from the Passport Agency. These fees can be combined with the passport book fee and paid by a single check or money order, payable to: U.S. Department of State. The optional return delivery fee is not required, although it is strongly recommended to receive the passport within the expedited processing timeframe.

  • In addition, you may pay an optional overnight shipping fee of $25.50 for faster delivery of your application to the Passport Agency to begin processing. This fee must be a separate payment in cash or credit/debit card. This fee is not required for expedited applications, although it is strongly recommended to receive the passport within the expedited processing timeframe.

PASSPORT CARD

The Passport Card is for travel by land or sea between the United States, Canada, Mexico, the Caribbean and Bermuda.


It is not valid for travel by air.

For a Passport Card, the applicant must appear in person and present the documentation required for a Passport Book.


  • Fees are $30.00 for adults, age 16 and over (valid for 10 years). 16 - 17 year olds must be accompanied by one parent who has proof of identity.

  • Fees are $15.00 for children, age 15 and under (valid for 5 years). See special requirements below for children under the age of 16.

  • Payable by check or money order to: US Department of State

  • A $35.00 execution fee, payable by a separate check, money order, cash or credit/debit card is due to the Clerk of Court.

  • Currently Passport Card processing takes 4-6 weeks.

  • You may pay an additional $60.00 to receive your expedited Passport Card in 2-3 weeks.

  • The passport fee ($30.00 or $15.00) and the expedited fee ($60.00) should be combined and paid by a single check or money order, payable to: U. S. Department of State.

SPECIAL REQUIREMENTS FOR CHILDREN UNDER AGE 16 (As directed by Public Law 106-113 and 22 CFR 51.28:)

To submit an application for a child under age 16, both parents or the child's legal guardian(s) and the child must appear and present all of the following:


  • Evidence of child's U.S. citizenship, which is a certified birth certificate or naturalization certificate, along with a photocopy, and previous passport if applicable, and

  • Evidence of child's relationship to parents or guardian(s), and

  • Proof of parental identity, I.E.; a valid driver's license, government ID card, along with a photocopy of front and back.

If only one parent appears for a child under age 16, you must also submit one of the following:


  • The non-applying parent must complete a statement of consent or special circumstances issuance for the minor child (Form DS-3053). This form must be notarized. The non-applying parent must provide a photocopy of the front and back of their pictured state/government identification.

  • Certified evidence of sole authority to apply.

  • A written statement (made under penalty of perjury) explaining the second parent's unavailability (Form DS-5525). This statement should include information on the absent parent such as: last known address; last known phone numbers; last place of employment; number and manner of attempts to locate.

RENEWALS

The applicant does not need to appear before an accepting agent to renew their passport book/card.

A convenience fee of 3.00% will be assessed to each credit/debit card transaction. There is a minimum convenience fee of $1.00 for all transactions under $33.00.
